DeathBan is a self explanatory, simple and lightweight plugin which bans a player when he dies.
Config

# Message shown to permanently banned players
ban-message: "You have been banned for dying!"
Commands
- /deathban exempt - Add player to exemption list
- /deathban unexempt - Remove player from exemption list
- /deathban list - See who's exempt
- /deathban reload - Reload config
- /reviveall - Unban all players at once
