This Resourcepack Is not intended to be used on it's own, however you can, so I'll just quickly tell you how to get the custom modeled items.
With the command: /give @s music_disc_creator[minecraft:custom_model_data=], with the replaced with a number, you can get a block staff!
Check the changelog for the valid values.
