DOOMED
A brutal, hands-on medical and survival simulation for Minecraft. Your body is real now - organs, blood types, bones, individual wounds - and healing is something you do with your own shaking hands. So is dying.

CONTENT WARNING - PLEASE READ BEFORE YOU INSTALL
Doomed is a work of horror fiction. It is built to be disturbing, and it depicts heavy subject matter openly and without flinching.
This mod contains, in explicit detail:
▪ Graphic injury and gore - bleeding, broken bones, amputation, shrapnel, blown-out eyes, wounds you treat by hand
▪ Sustained medical distress - pain, suffocation, cardiac failure, brain damage, sickness, hands-on treatment you can fail ▪ Drug synthesis, use, addiction and withdrawal - opioids included (possible to disable in config) - depicted with their costs
▪ Cannibalism, with a disease you can only catch from it
▪ Despair, and self-harm themes. A character in a very low mental state can enter a self-harm episode; the story's climax is a despair-driven, unskippable transformation-death sequence in which the character confronts their own self-destruction.
This content is fiction. It depicts a doomed character's descent and self-destruction as horror, in the tradition of Project Zomboid and Casualties Unknown (this mod is not affiliated with either). It is not instructional, and it is not an endorsement or encouragement of self-harm. Every dark line you read in-game is spoken by the fiction - the curse, the entity, the character's own breaking mind - never as advice to you.

Server owners can disable individual Doomed systems in the config (despair, frostbite, opioids, infection, radiation, trap damage, and more), and any player can leave the whole thing behind with /doom off. You are never trapped in it.
⚠ AND IT IS NOT A DIFFICULTY SETTING ⚠
Suffering is the subject, not a side effect. You are cursed with a fragile body. You will bleed out. You will break bones, choke on your own blood, catch infections, lose your mind to brain damage, and shake so badly you can barely treat yourself. Healing is not a hotbar item you click - it is a set of tactile, fiddly, hands-on minigames you perform on your own flesh while in agony, and you can absolutely botch them and make things worse.
Suffer well.
DOOMED makes Minecraft significantly harder to survive.
Doomed does not modify your health bar. It replaces it with a body: organs that fail, blood with a type, bones and joints and a spine, and wounds that are each their own injury with their own depth, contamination and age. Over 280 items, and most of them are ways to keep that body going. It is opt-in per player and built for multiplayer - and friends who never type /doom stay vanilla.
You can choose to become Doomed
Simply input '/doom' in the chat
Doing so marks you as something the world is allowed to hate. Compatible mobs and systems may become more hostile toward you; soft integrations can treat you as a target. And something begins to speak. It does not stop.

The short version
- You have organs now. They fail, they can be transplanted, and the replacement is worse than the original in ways you get to discover.
- Medicine is a set of hands-on procedures performed with a hand that shakes with your pain.
- A friend can perform almost all of them on you, with steadier hands than yours.
- Some deep caves have grown into something that watches back.
- Choosing the transformation has an ending. It cannot be skipped.
The Body
Press H to open up your health panel.
- Pain, shock, consciousness, oxygen, brain function, heart rhythm, blood pressure and core temperature - tracked separately! It's a huge system you will have to navigate.
- Organs, with the medicine to match: failure cascades, transplantation, donor tissue that is never quite yours
- Every wound is its own injury - depth, bleeding, contamination, dressing and age.
- Bones, joints and a spine. A dislocated jaw will not chew. An unstable spine cannot be splinted.
- Eight real blood types, assigned for life. The wrong transfusion is worse than nothing - a delayed reaction that destroys more blood than the bag ever held.
- Amputation has an afterlife: stump healing, phantom pain, socket irritation, and three tiers of prosthetics that wear down, need lubricant, but will never be able to replace what you once had.
- Diseases with real stages - including one you can only catch from eating people. Cooking does not make it okay
- 75 moodles tell you, at a glance, what is going wrong
- An ordinary hit can never delete you - damage becomes wounds, and the body decides what they cost. Force past survivable, a heavy round to a bare skull or a naval shell, kills outright - and even that has an off switch in the config! Woah! Truly amazing, donate me alllll your money :D
Treatment
Every procedure is performed with an in-fiction hand that trembles with your pain and tints red as it climbs; when you are weak, it drifts. Treating while hurt genuinely makes you worse at treating - which is the entire problem, and the reason friends matter.
- Bandaging, suturing, splinting, joint reduction, shrapnel and arrow extraction, injection, defibrillation, CPR, cauterization, direct pressure, amputation - each a distinct hands-on minigame with its own failure modes
- Overtighten a splint and the splint is a tourniquet. A real tourniquet buys about nine safe minutes; around fourteen, the limb is dead tissue..
- Direct pressure needs nothing at all: both bare hands on the wound, stamina draining, locked into a screen that does not pause while whatever cut you is still nearby
- The early game is herbalism - a foraging knife, wild herbs by climate, a pestle and a stir. Weaker than real medicine on purpose. We'll have to make due though!
- Prefer the medicine without the hands? A config option resolves the dexterity minigames in one click at the same item cost and rules - CPR, cauterization, direct pressure and amputation are always performed for real
Bring a Friend
Much of the medicine is easier with two people. Some of it is impossible alone.
- Nearly every treatment works on another player through the same real minigames - and it is the helper's pain that shakes the hand, not the patient's. A healthy friend has the steady hands your mangled ones cannot manage!
- Consent is given with the body: crouch to refuse, hold still to allow. An unconscious patient counts as allowing - that femoral bleed is what the system exists for! Fancy words, love to see it. Get used to it too!
- CPR is empty-handed: compressions at 100-120 a minute buy brain time and nothing else. The heart does not restart because you pressed on it. Better get a defib unit asap.
- Draw 0.75 L of blood into a labelled, typed bag - your own or a teammate's. Match the type.
- Scan a patient with the WITNESS tablet: triage, findings, and a body target for your next procedure.
- You can amputate a teammate's ruined limb. If it is salvageable, they must be awake and agreeing.
What Dying Leaves Behind
The server writes down what happens to you while it is happening. When it ends, the death screen prints it back in order - Hold TAB for the whole file.
- Underneath sits a closure line: "Closure: sutured, Left Hand, 7m 12s." or "Closure: none."
- Every death also writes a plain-text case file into a doomed-morgue folder in your game directory, piling up across every world and server you ever play.

Survival Layer

- Temperature. Weather, night, altitude and shelter drive your core body temperature, with real thermal mass - the coat you put on before you set out matters more than the one you put on already freezing. An exposed night can frostbite off your fingers; heat sources stack with diminishing returns, so a room you built can actually be warm.
- Warm up. Campfires, lava and lit furnaces warm you through your clothing - plus the fuel-burning Brazier (feed it a netherite block and it burns forever) and RF-powered Heater / Cooler machines. Other mods' heaters and coolers work too, via datapack block tags.
- Gear up. Sun-ferns and frost-ferns drop fibre for craftable insulation - fur coat, liner, hood - plus a warming Hot Drink, a chilling Cold Pack, and a Thermometer for an exact readout on your HUD.
- Water. Fill a Canteen from open water and boil it clean on a fire, brazier or heater - or drink straight from open water with an empty hand, if you don't mind the silt (and the sickness that comes with it).
- Food rots. Cold biomes slow it, deserts hurry it! The powered Freezer stops time inside; the Icebox burns snow and ice to slow it down. Blood bags spoil too - cold storage is medical infrastructure.
- Nutrition is real. Food restores actual nutrient stores sized by the portion, and iron is the one that will kill you - blood is remade out of it, and every litre you lose spends it. Pam's HarvestCraft, Croptopia and Farmer's Delight are already covered.
- Sleep quality, hygiene, weight, wetness. A bare-floor nap is not a comfy bed! Grime suppresses immunity and infects open wounds - wash stations, lye soap and wet wipes exist for a reason. Barefoot walking can leave a friend in your feet as well..
The Industry of Staying Alive
- One power block: the Fuel Generator, burning ordinary furnace fuel.
- Nine chemistry machines and a Reactor run real multi-step synthesis - poppies to latex to opium to morphine, and onward into things you should not make. Purity is rolled from how well you actually ran the step, and a dirty batch carries its filth into the syringe.
- Chemistry is dangerous the way it should be: fumes sicken you without a mask, flammable steps can flash over open flame, sealed reactions can rupture, and dirty glassware taints the next batch.
- Machines take per-face routing and real heat staging - a brazier under the vessel is tier one, more heaters against it go higher, and too much heat overcooks the batch. Every recipe opens natively in JEI and EMI.
- Twenty-six drugs exist as real, pipeable Forge fluids with purity tracked in the tank - your tech mod's pipes can move morphine.
- Sealed supply pods cracked through a staged defusal on a 180-second clock you cannot pause or reset - walking away or relogging does not stop it. Every threat announces itself first - a hiss, or a blink, or scorching on the hull.
- Batteries come in real sizes, rebreathers refill at the Oxygen Compressor, and the Decon Station scrubs and disinfects everything at once

The World Is Rigged
- Traps generate by depth, denser and nastier the deeper you go: spike traps and caltrops up top, live wires, sawblades and gunmines further down, landmines and the electrified coil in the deep.
- Surface minefields. Crystal caverns that are radioactive, or unstable, or both.
- Locked crates open with a lockpicking minigame if you are Doomed.
- Landmines can be defused: watch the circuits, cut the steady wire. The wrong wire is also available! Not recommended though, success pays salvage and, rarely, a cookie
- Buried trader lairs hold live merchants, the maid's medical hints are dependable, the adventurer's deliberately are not. You must be Doomed to trade!
- Grabber plants swing you like a toy. Walls stop them. Fire ends them!
The Cyst
Rare deep caves have grown into something else: hundred-block jungle caverns of undergrowth, root arches, spore blooms and water you should not drink. The walls grow eyes that sweep spotlights through the dark - the only light down there, which is the trap, because you will want to navigate by them. The ears hear through stone, and sprinting is twice as loud as walking. Every grove keeps exactly one Deep Maw: a persistent resident that hunts the caves in three dimensions, treats your light as an invite to a meal, digs through rock to reach you, and grabs you by a limb. You can tear free at the cost of a laceration. Or you can give it the limb, and leave while it feeds. That is a genuine decision, and you have a few seconds to make it.
- The grove pays: rooted casualties to search, a locked medical cache - blood, defibrillators, medicine you cannot craft - and one larger ruin apiece!
- Gall berries are the only real food down there. Cook them, or carry the spores home in your lungs! Not recommended...
- Burning an eye puts it out quietly - but the smoke is toxic, the smell carries, and a half-cleared grove grows back.
- Kill the Maw and it seals itself into the rock as a scar. The grove is yours after that, for whatever that is worth...
The WITNESS Tablet
Craftable early, no machinery needed. Live ECG, blood pressure, oxygen, blood volume with loss per minute - and a diagnosis engine that names the root problem, nests its complications under it, ranks the most urgent intervention first, and admits what it cannot know. Treat our AI girl well, please!
Out of the box it is not your tablet: it boots as a UNI-HEALTH(tm) device, files you as an ASSET with a number, runs a sponsored ad band over your dying body, pops up a satisfaction survey the moment you stabilize, and auto-accepts a EULA on your behalf. Clause 22: "By continuing to live, you accept these terms."
IRIS is built into the device. She is not always nice, but she is usually correct.
"IRIS: Do not rate that five stars." - over the pop-up asking you to rate your critical incident
- Stay alive twelve hours in one life and IRIS can chip through the Bureau's lock - all but the last step, which is physical, and yours... enjoy the spoils!
- The tablet's field manual indexes only what your character has actually lived: conditions unlock by surviving them, items by holding them, and learned pages survive death
- A Field Journal item writes itself, one page at a time, from things that really happened to you - each entry ending in a rough graphite sketch. Inspired by Darkwooooooddd!!!
The Transformation
One question at the start: Endless, or C.A. #DR-01. Changing your answer later costs a death.
Endless is the health simulation, indefinitely, with a trait screen - perks paid for with flaws, starting budget zero.
C.A. #DR-01 is the other thing. One of these has an ending.
- Traits, for the Endless: Claws that blunt from mining. Fur that is the warmest thing you can own until it is soaked. Missing Left Arm, which is exactly what it says until you fit a prosthetic. SEVERED, which takes away the panel that explains your body and leaves you to look and touch and smell for yourself - many great perks truly truly!!
- One experiment: Schizophrenia - an imaginary friend who talks to you when you bleed and warns you of large things moving nearby. Wrong about one time in five. In both directions Still being worked on since 23/08/2026 but it will be finished in two weeks since this date I hope.
The Ending
The transformation has an ending.
(Headphones recommended. Flashing-image / photosensitivity warning applies - the sequence opens on a warning card and honors your intensity settings.)
Survivors carry permanent marks - and permanent gifts.
Accessibility
Doomed is a horror mod that intends to be playable by people the horror genre usually ignores.
- Screen distortion turns down to zero - one master vision slider, with separate controls for static, screen shake, illness camera motion, spore fog and searchlight glare. At zero, your moodles and health panel still tell you everything. Turning effects down never changes the simulation.
- Sound is split so you can turn things down without losing the warnings: pain vocalizations mute separately from survival cues, the dying sounds carry captions, and no slider can silence the flatline.
- One-click treatment mode skips the dexterity minigames at the same cost and rules.
- Choose how eye loss is drawn. Freeze the tablet's decorative motion. In VR, illness stops grabbing your head - the afflictions still happen, they just no longer own your neck.
For Modpack Authors & Server Owners
- Over 200 config options, and a standing contract: every heavy system - temperature, frostbite, despair, infection, radiation, organs, contagion, fumes, spoilage, traps, gibbing, the Cyst, the Maw - switches off mid-world without bricking the save - a body stuck mid-crisis is healed by the toggle, not stranded by it - and can be switched back on later without a wipe.
- /doom preset clinic flips off the survival pressure and keeps every medical system live: a sandbox for practising medicine, not god mode.
- One pace factor scales every vital rate from 0.05x to 100x, with per-system multipliers - bleed, hunger, thirst, infection, spoilage - each settable to zero.
- Pack hooks throughout: heat and cold source tags, hydrating food tags and damage-type physiology profiles (poison finds the liver, dragon breath finds the lungs) in datapacks; fully editable loot lists and dimension allow/deny policies in the server config.
- Onboarding is yours: Doomed is opt-in by command, or a pack can auto-open the ritual on first join, or silently enroll everyone.
- Servers with a decided look can lock individual customization options without silently disabling mechanics.
- Any player can renounce the class entirely with
/doom off. You are NEVER trapped in it.

- JEI and EMI browse every chemistry, grinding and machine recipe natively; Jade reads heat tier, run state and glassware condition off the block!
- Fantastic RPG Class System: Doomed appears as a selectable path.
