Mental health becomes a stat to manage with Sanity: Descent Into Madness, an add-on that adds a sanity mechanic inspired by a popular survival game. Experimental Features must be enabled for it to work.
As sanity drops, the screen grows muddy and blurry, hallucinated sounds start to play, and inner entities born from the unstable mind appear and attack the player. Some existing mobs also behave differently, and neglecting sanity can even be fatal. Players can restore it in various ways, and a garland crafted from small flowers gives a temporary buffer against the madness. The Rotting Stalker and Sneaking Terror are among the new entries.
